When you come upon Antie Ethel's swamp and uncover her true identity as the Hag, she offers you a trade: the extraction of your parasite in exchange for an eye.
The first time you have the chance to meet Antie Ethel is in the refugee camp. If you're a magic user, you might be able to sense there's something off with her before you even go to her sunlit grove, which is actually a dank swamp, and those sheep ain't sheep.
Follow the questline and you'll find Antie Ethel inside her hut, where she's holding a local pregnant woman captive. At this point, she'll offer you the trade: removal of your tadpole in exchange for one of your eyes.
Baldur's Gate 3 - should you give the Hag your eye?
If you're actually looking to remove your tadpole, you should know that this choice won't actually do that. She realizes that yours is special and refuses to work with it. Unfortunately, at this point, your eye is already gone and replaced with a magical green eye.
Your new eye gives you +1 on intimidation, but a disadvantage on perception checks and attacks against hags. Perception is an important stat in Baldur's Gate 3, so it depends on whether or not your party can pick up the slack or not. You'll also get a potion that's a one-time use, which gives you an advantage on all checks until your next long rest.
Choosing to have your eye taken out can greatly impact your game. For our run, we decided not to go along with Auntie Ethel's plan. It's important to note that most players have reported that not letting Ethel take your eye will leave the quest incomplete in your journal until you head to Act Two.
It also makes attacks on hags have disadvantage, so deciding to go to battle with Auntie Ethel after this point will result in an even tougher fight than before. Most importantly, Auntie Ethel aborts the removal of the parasite, rendering the main draw of helping her in Baldur's Gate 3 moot.
The first time you meet Auntie Ethel, she'll offer to cure you of your parasite - do not take this deal! She just takes your eye, permanently giving you a stat penalty and not curing you of anything. However, if you can lower her health significantly in the boss fight without killing her, she offers you a new deal.
Ethel started bargaining for her life. She offered me power (AKA, an ability point) if I let her live and gave her Mayrina. I could negotiate and offer just Mayrina's unborn child, or persuade Ethel to give me the ability point and Mayrina while being happy I let her live.
Auntie Ethel is a minor antagonist and boss in the 2023 fantasy video game, Baldur's Gate 3, serving as the titular main antagonist of the Hag storyline. She is a cruel and sad*stic Green Hag (a race of witch monsters) who disguises herself as a kind old merchant to lure and capture victims for her experiments.
But she should be alive as long as the cage is not on fire. It's not timed. Many people don't know this, but you can catch Ethel invisible there and kill her if you can burst her in a round.
To get this buff, players will have to fight Auntie Ethel in Act One, saving her victim, Mayrina. When reduced to low health, Auntie Ethel will use a turn to bargain with the players, offering this buff in exchange for her life and her hostage.
It may seem counterintuitive, but the Ersatz Eye Volo provides in exchange actually does Tav far more good than their organic right eye ever could. The see invisibility effect doesn't work 100% of the time - enemies still need to fail a roll in order to be revealed - but when it does, it's a major boon.
It's worth noting that the same character cannot have both Volo's Ersatz Eye and the Hag's Eye from Auntie Ethel. This shouldn't be a huge issue, as the Hag's Eye effect is not worth its disadvantages, it's just something to consider if the character has already given her their eye.
He'll panic and give you a magical prosthetic eye before fleeing. The prosthetic eye has an enchantment that lets you See Invisibility, a permanent upgrade that can be very useful. You'll be able to automatically see invisible creatures and objects for the rest of the game.
Summary. Letting Volo perform the surgery in Baldur's Gate 3 results in companion disapproval, but it gives the player a prosthesis called Volo's Ersatz Eye. The Ersatz Eye grants the effects of the see invisibility spell, but also comes with a temporary debuff.
Although they will make Ethel's fight much harder if you ignore them, saving them may be the moral decision. If you want to make the fight easier and also save them, knock them unconscious but refuse to take off their masks. Otherwise, their minds will collapse, and they will die.
If the party killed Ethel, they can still work with her by finding her disguised as Captain Grisly during Save Vanra. If the party kills Lora, then Ethel is willing to strike a deal with them, even if they killed her previously, as she is so amused by the turn of events.
As should be expected, almost all of these potions are poisons in disguise, some of which can destroy a character build or result in a party wipe. A Mother's Loathing and Heart of Stone are situationally useful for a while but neither one grants a permanent buff.
